What to include in a notice

A valid notice under 17 U.S.C. § 512(c)(3) must include: identification of the copyrighted work, identification of the allegedly infringing material with enough detail to locate it, your contact information, a good-faith statement, a statement under penalty of perjury that you are authorized to act, and your signature.

Counter-notice

If you believe your content was removed in error, you may submit a counter-notice under 17 U.S.C. § 512(g)(3). A counter-notice must include: identification of the removed material, a statement under penalty of perjury that you have a good-faith belief the removal was a mistake or misidentification, your contact information, and consent to the jurisdiction of federal court.
